Perpetua Longevity ("Perpetua," "we," "us," or "our") respects your privacy. This Privacy Policy explains how we collect, use, disclose, and safeguard information you provide when visiting our website or engaging with our virtual concierge medical practice.

Information We Collect

We collect information you submit through our membership application and inquiry forms, including your name, email, phone number, state of residence, age, current medications, and health goals. When you become a patient, we also collect protected health information (PHI) as required to provide medical care.

How We Use Your Information

Information is used to respond to inquiries, evaluate membership applications, deliver and coordinate medical care, schedule appointments, communicate with you about your care, and meet legal and regulatory obligations.

HIPAA

As a healthcare provider, we comply with the Health Insurance Portability and Accountability Act (HIPAA). PHI is protected by separate Notices of Privacy Practices provided at the start of care.

Sharing

We do not sell your personal information. We share information only with qualified third-party providers (such as laboratories, imaging centers, and pharmacies) required to deliver care you have requested, and as otherwise required by law.

Security

We use industry-standard technical and administrative safeguards to protect your information, including encryption in transit. No method of transmission is 100% secure.
